The Institute offers a unique opportunity for patients to receive all their treatment in a totally outpatient basis. Dr. Maharaj has been performing bone marrow / stem cell transplants since 1995. He has demonstrated that the majority of patients can be treated safely with minimal complications on a totally outpatient basis. This has been achieved by close monitoring and early intervention through all phases of the treatment by the same staff.

WHY THE OUTPATIENT TREATMENT?

Patients with a diagnosis of myeloid leukemia are up to 66 times at greater risk of sepsis (infection throughout their body) during inpatient treatment compared to patients who are admitted for a non-cancer diagnosis. This data comes from a study of over 600,000 patients from 6 states (including Florida), which was published in the Critical Care Journal in 2004.

This risk is further increased because of the high incidence of hospital-acquired infections that are prevalent. Patients receiving intermediate or high-dose chemotherapy have an additional layer of risk because of the severe immunosuppressive effects of the chemotherapy.

One of the primary objectives of the Institute has been to reduce the incidence of infections and this is being successfully achieved. The second objective is the reduction of the toxicities associated with intermediate and high-dose chemotherapy treatment. At the Institute this is achieved by proactive interventions. For example, hand washing is strictly adhered to, prophylactic medications are used to prevent the known toxicities of the chemotherapy, close monitoring and evaluations allow early intervention and treatment to prevent in-patient hospitalizations. In contrast, typically inpatient hospital transplant care is reactive, with treatment of the complications as they arise which results in additional morbidity and treatment to the patient and prolonged hospitalization. Reducing or eliminating hospital admissions also results in significant cost savings to patients, payers and employers.

The Institute provides close monitoring and treatment by the same healthcare professionals through all phases of treatment; whereas hospital care is usually done by different staff, e.g. typically attending physicians rotate on a weekly or monthly basis. Since this proactive approach decreases or eliminates complications, the need for multiple consultant physicians and staff attending to treat the patient is also decreased.

Patients who receive a totally outpatient bone marrow/stem cell transplant return home following treatments and remain with family and loved ones. As one of our patients told us, this can be especially comforting because “you don’t feel as sick when you are at home compared to when you’re in the hospital”. By avoiding hospitalization, patients are also able to return to work or to resume normal daily activities sooner.

We have achieved excellent results with our program. The "day 100" post-transplant mortality rate and patient success rate of being alive and disease free match or exceed those of nationally known transplant centers. The incidence of infections in our program is significantly lower than at inpatient transplant centers. That says a lot about trusted, quality care that you can feel good about.
